Referrals must be new patients who haven’t previously booked at 365 Health.
Excludes Parents & Children

Spouses, siblings, friends, coworkers and extended family all qualify — parents and children of the referring patient do not.
Credit on First Booking

Your credit is applied once your referral completes their first booked appointment.

Points are tracked for you — ask our front desk for your current balance any time.
Reward Substitutions
